1. First print out the Patient Order Form.
   
   
2. Then use the drug search page to get the prices for the drugs you need.
   
   
3. Attach a copy of your prescription (keep the original) to the form in the space provided.
   Many doctors prefer to fax your prescription directly to the pharmacy.
   
   
4. Mail or fax that form to the pharmacy and that's all there is to it.

When you need a refill, on your current prescription, you can request it over the phone or online. If you prefer, you can also fill out the order form and send it in without the prescription. When you have a current prescription on file with refills remaining, you don't need to send in another one.

If you would like the assurance of talking to a real person you can call ahead and set up your account, on the phone, before you mail the form.
